This form should only be used by applicants who are not existing members of the Association.
 

Status Declaration Form

Professional and Qualified membership is assigned either a Practicing or Non-practicing Status depending on whether or not you have met VAHTC or ACA targets for Professional Development and Professional Supervision.
 

Membership Upgrade Form

Use this form to upgrade your membership category. Phoenix students whose classes finish in December will normally be upgraded automatically from Student to Qualified as part of the membership renewal process.

Use this form to update your contact details; your postal or email address and contact phone numbers.
